This is the full length assessment, which allows for a more in depth look at what’s going on at the school. This will help to determine a specific area to focus on. It is arranged according to the aspects of the Healthy Living Performance Standards, and also takes into account the 4 pillars of CSH. Detailed instructions for use are provided in the assessment tool.
How to use it:
School/School District: This assessment is more appropriate to be completed by a healthy school team or individuals who are working at the whole school or school district level. You may want to consider getting your local health authority rep involved as they may be able to provide assistance in completing the assessment.

Express Healthy Schools Assessment Tool
This assessment tool serves as an express version of the full length Healthy Schools Assessment Tool. It was created to help save time while still providing the opportunity to look at what is currently going on in the school to help determine an area of focus.
The express assessment is arranged according to the aspects of the HLPS. The questions provided are intended to help start the conversation around current activities within the school to determine where strengths are as well as areas that may need more attention.
How to use it:
School/School District: This assessment is similar to the full length Healthy Schools Assessment Tool in that it is more appropriate to be completed by a healthy schools team or individuals who are working at the whole school or school district level.
Once you determine an area of focus, you may refer to the full version of the Healthy Schools Assessment Tool to further refine your focus by asking more in-depth assessment questions.

Healthy Living Performance Standards Matrix
This assessment tool provides a quick, comprehensive snap shot of where you are at. By using guiding questions, it helps to assess your level of activity according to the aspects of the Healthy Living Performance Standards (HLPS) as well as the pillars of Comprehensive School Health (CSH).
The intention of the HLPS Matrix is to provide you with a quick comprehensive view of where your strengths are and an idea of where you could focus your efforts.
How to use it:
School/School District: This assessment can be completed by the teacher or as an activity with the whole class. If it is done with the class, it is recommendation that the teacher facilitates the process. Under each aspect of the HLPS, there are questions to consider in each pillar of CSH. These questions are meant to help start the conversation about current activities that are happening within the school/classroom in health HLPS aspect and CSH pillar.
